Overview:
At the request of the American Board of Preventive Medicine (ABPM), ACOEM has developed downloadable and fillable .pdf to assist you in completing MOC Part 4 requirements

This .pdf will guide you through completion of the required practice performance self-assessment process: review of your current practice performance; identification of an area in your practice that you will target for improvement; development and implementation of a quality improvement plan; and a second assessment to verify improvement of your practice performance.

How long will it take?
While the time required to complete the activities are minimal, the program should be completed over a minimum of a 6-month time period. Please note that the review process of completed modules may take up to an additional 60 days.

How do I submit my materials?
All submissions MUST be sent by email and must be complete (the fully executed fillable .pdf file with all required supporting documentation) in ONE email. All submissions should be emailed to: moc@acoem.org
Each form/booklet can be used for only one project. If you wish to submit another project, you will need to purchase another form/booklet.

Outcome:
Completion of this program will result in fulfilling the Part 4 component of the MAINTENANCE OF CERTIFICATION© requirements. This does not apply to the other three component parts as administered by the American Board of Preventive Medicine.
