An Innovative Approach to Weaning Opioid Dependent Workers: The MA Department of Industrial Accident Opioid Alternative Treatment Pathway
Despite a slight decline in opioid prescribing since 2012, significant gaps remain in opioid prescribing for pain. Long term opioid prescription may occur without exhausting less harmful alternative treatments, and may lack appropriate screening and monitoring. Causes include prescriber knowledge and practice gaps, unrealistic patient perceptions regarding opioid benefits vs. risks, and system barriers including insurance coverage for non-opioid treatments including behavioral health. Unfortunately, most patients prescribed opioids for chronic pain fail to experience significant pain or function improvement. Yet these patients have increased risks for aberrant opioid use, addiction, mood disorder, overdose and disability while insurers are faced with increased costs. In contrast, there is evidence that opioid weaning with a functional restoration approach can improve pain and function. This session includes a review of evidence that opioid weaning can improve outcomes, and discussion of a novel Massachusetts Dept. of Industrial Accidents multi-stakeholder collaborative intervention to assist patients who wish to wean off chronic opioids
1. Recite evidence regarding efficacy of non-opioid alternatives for chronic pain.
2. Review evidence regarding outcomes for long term opioid users after opioid weaning combined with functional restoration approaches.
3. Describe a novel opioid alternative treatment pathway program from MA DIA including negotiations among all stakeholders
