Fitness for Certification of the Merchant Mariner: Fact, Myth and Inuendo

Session Description:
The Coast Guard is the government agency charged with providing safety for the United States Marine Transportation System (MTS). A critical component of the MTS is the merchant marine workforce, a group of dedicated maritime professionals employed on the fleet of marine vessels. A mariner obtains certification by submitting an application completed by a medical provider, then submitting the application and required documentation to one of the Coast Guard’s 17 Regional Examination Centers (RECs), or directly to the Medical Evaluation Division at the National Maritime Center. In 2015, reform of the medical certification process was mandated by a Congressional Coast Guard Authorization Act which required the establishment of a Designated Medical Examiner Program (DMEP) to have a "trusted agent" who could issue a medical certificate to qualified mariners. This session will increase the understanding of practicing OEM physicians of the fitness-for-certification process for merchant mariners. It will include an overview of current regulations and guidance on completing the medical certificate application, and an update on the status of the DMEP.

Learning Objectives:
1. Recognize the primary objective of determining a mariner's fitness for certification.
2. Identify the scope of documentation on the general medical exam required for the CG 719K merchant mariner application.
3. Distinguish between standards versus guidance outlined in the Merchant Mariner Medical Manual, Commandant Instruction Manual 16721.48.

Target Audience
Physicians who specialize in or have an interest in OEM as well as for non-physicians, such as industrial hygienists, nurses, safety professionals, and environmental health specialists who are involved in the field.
